First, be sure to collect information prior to completing the form (i.e. Contact Information for Emergencies, Doctor's Name, Insurance Carrier and Policy Number, a Personal Email Address) because this process does not save a partial submission.

Use the Tab Button to move from one question to the next. If you hit the Enter button, you will submit the form and due to the required questions, error messages will come up on all necessary questions.

Regarding the required Athletic Policies, you will only need to view and read them (you no longer need to print out the policies and sign). Then hit "SUBMIT" at the end of this form to send a confirmation email to your email address.  Print and sign that email, and then turn your email confirmation into the Finance Office to be stamped.
